4 Easy steps to get started

Step 1

We receive your referral, gather necessary documents from your case manager and set up a call with you to go over what you want in the plan.

Step 2

After connecting, we take the information we've gathered and write the CDCS plan.

Step 3

Once the CDCS plan is completed, we send it over to you for review.

Step 4

After you approve the draft, we send it to the case manager for approval or for any changes that need to be made.

Our Story

We have been friends for what seems like forever! It all started in 2016 when Taylor began her internship at the same place Abby was working. Fate brought us together when Taylor was assigned as Abby’s’ direct intern.

WE CONNECTED INSTANTLY

We worked great together as we had similar habits and values. Taylor’s internship quickly became her full-time job and we worked together at this location for 2 years. In these two years, our friendship grew tremendously. We became very close with each other and each other’s families.
